1. Make an appointment for a Tour of Eden Care Center.
2. Complete Enrollment Packet (Please be sure to bring in: Your child’s Physical done by the child’s Physician within 30 days, Medi-Cal & All Insurance Cards for child).
3. Our Registered Nurse will do a Nursing Assessment on your child and create a Plan of Treatment (POT)
4. Eden Care will submit the POT to Medi-Cal and as soon as it is approved, your child is enrolled!
